Herein this document the “Independent Filmmaker” is called the “Producer”.

References in this document to the “Record” shall include the physical media, the electronic and magnetic images thereon, and the recorded audio and video content.

WHEREAS The Producer wishes to be able to include in the Record portions or all of photographic, video and/ audio recordings made of persons present, one of whom the undersigned Audience Participant, wishes to be, and the Producer will not admit to the Location of the Shoot any person who has not consented to the provisions of this document.

WHEREAS, the Audience Participant requests admission to the above named Project and consents to the provisions of this agreement (the “Agreement”).

The Audience Participant hereby makes the following acts, representations and promises to the Producer:

1. I represent that I am over the age of 18 years and that I have the legal capacity to contract on my own behalf.

2. I consent in advance to the filming, audio and/or video recording by any means (the “Record”) of the Project, including (without limitation thereto) my own attendance and participation at the Project, including all extensions and postponements thereof, regardless of venue

3. The Audience Participation must not record the Project by any means whatsoever. If at the Project I, the Audience Participant, undertake to operate any recording equipment, I shall immediately thereafter deliver to the Producer the original and all copies . . . (continued)
