Herein the Independent Filmmaker is called the “Producer”.

1. In Consideration for allowing the above named and below signed Project Participant, (hereafter called the “Participant”), to participate in and/or be a part of the above named Project, the Participant hereby agrees, on behalf of the Participant and on behalf of the Participant’s personal representatives, heirs, and next of kin, as follows:

The Participant hereby grants the above named and below signed Producer the irrevocable Right (but not the obligation) to record, exhibit and otherwise use the Participant’s name, likeness, photograph, voice, sounds, and biographical data in any and all of the Film Footage and Still Photographs, both on and in connection with the Project as well as in any materials used to advertise, promote and publicize the Project.

3. The Participant has the power and authority to, and hereby, grant to the Producer(s) the irrevocable right (but not the obligation) to record, exhibit and otherwise use the Participant’s likeness, photograph, film or pictorial rendition of the Premises in the Film Footage and/or Still Photographs or otherwise in connection with the Project as well as in any materials used to advertise, promote and publicize the Project.

4. If the Participant does not have the power and authority to grant the foregoing rights with respect to the Premises, the Participant have provided the Producer(s) with the name, address and telephone number of the person or parties with such power and authority.

5. The Participant understands, acknowledges and agrees that the Producer(s) is and shall be the sole and exclusive owner of the Project, including without limitations, the titles thereof, the Film Footage, the Still Photographs and all rights (including copyrights), components and elements contained therein, for all purposes and uses whatsoever, throughout the universe . . .
